A Blazor web application for tracking mental health through standardized questionnaires including GAD-7 (Anxiety), PHQ-9 (Depression), MDQ (Bipolar), and ADHD assessments.

Questionnaires Included:

GAD-7 - Generalized Anxiety Disorder (7 questions, 0-21 score)
PHQ-9 - Patient Health Questionnaire for Depression (9 questions, 0-27 score)
MDQ - Mood Disorder Questionnaire for Bipolar (13 questions, 0-13 score)
ADHD - Inattentive & Hyperactive assessments (18 questions total)
